There are two forms, one is the main form with your contact information and a count of the skins you are sending in. Please check the proper category and give us any special instructions on that sheet. The second is a listing form. If you have multiple skins with names/numbers just fill in the lines so we can get a detailed count of your deer hides or other species.

Please place all forms in either water proof bags, or taped to the outside of your box. We take the time to keep your order sorted and correct.
